What to do when there is a Workers Compensation claim?

Workers Compensation policy with Farmers Insurance:

If it is an emergency, sent the injured employee for treatment right away.
Call Farmers’ Workers Compensation claim department at 925-847-3141 to report the claim.
Have the injured employee and store manager to fill up these 2 claim forms (Form 1 and Form 2), keep a filled-up copy, and fax it to Farmers Insurance at 925-416-0948.

Workers Compensation policy with California State Compensation Insurance Fund:

If it is an emergency, sent the injured employee for treatment right away.
Call State Fund Workers Compensation claim department at 888-222-3211 to report the claim.
Have the injured employee and store manager to fill up these 2 claim forms (Form 1 and Form 2), keep a filled-up copy, and fax it to State Fund at 800-371-5905.

Workers Compensation policy with other insurance company through Zain Jeewanjee Insurance Agency.
